Physical Therapy Evaluation
Please note that there is a five-year limit on requests for copies of any evaluation. We are currently able to issue duplicate copies of reports dated 2018 and thereafter. For evaluations issued prior to 2018, a new report and new application fees are required. IERF reserves the right to deny a request for a duplicate copy on the basis of changes that may have taken place in:
- the requirements of the licensing board to which the report is to be sent, since the time the initial report was issued
- the educational system of the country of study
- IERF policy
There is also a five-year limit on requests for revisions. Revisions include adding new studies, changing the report for another state board, and changing the report for a different level of licensure, if allowed by the board (e.g., PT vs. PTA licensure). We are currently able to issue revisions of reports dated 2018 and thereafter. For evaluations issued prior to 2018, a new report and new application fees are required. Whether your request is for a duplicate or a revision, please note that, depending on the state board, additional or new documents might be required. Upon receipt of your application and payment, your file will be reviewed and you will be notified accordingly. A different form is required for services other than duplicates/revisions of physical therapy evaluations.

A delivery option must also be selected for each official report requested, whether a duplicate or revision, and the corresponding fees will be applied (see mailing options below).
Duplicate Report
Description | Duplicates are only available for the same state/type of licensure (PT or PTA), with no additional studies or modifications. Existing evaluation reports may be modified to reflect current individual state regulations. |
Fee | One official sealed board copy and one applicant copy are included in the fee.

Processing Time | There are no rush services for physical therapy evaluations. Please note that the processing time for the evaluation only begins once we have received all required documentation (and/or fees). Allow a minimum of 3 weeks from the receipt of all required documentation (and/or fees) for your evaluation to be mailed. |
Revised Report
Description | Revisions include reports for new states, new types of licensure (PT or PTA), and/or for additional studies. Existing evaluation reports may be modified to reflect current individual state regulations. |
Fee | One official sealed board copy and one applicant copy are included in the fee.

Mailing Options
As required by all state boards (except California), your evaluation will be addressed and mailed directly to the board. If your state board accepts evaluations by email, you may select the email option and provide their email address in the mailing instructions section of IERF’s online application. The Official Report Delivery Fees are as follows (only one delivery option is available per evaluation report):
- Each Electronic Delivery for $10 per transmission
- Regular Mail for $10 per address
- Domestic Secure Mailing for $25 per physical address in the continental U.S.
- Domestic Next Day Delivery for $40 per physical address in the continental U.S.
- International Secure Mailing for $75 per physical address (if additional fees apply, you will be contacted)
If submitting original documents, we recommend that you select either Secure Mailing or Domestic Next Day Delivery for return of your academic records, as these include a tracking number. Please note that the original documents that you submit will not be returned to you via regular mail unless you instruct us in writing to do so. IERF accepts no liability related to the loss or damage of documents during mailing.
Payments
Payment in the form of a check, cashier’s check, money order or credit card must accompany all applications, and must be payable to IERF. Overseas payments must be in U.S. dollars and drawn on a United States bank. If payment is being made by a bank draft, an additional $15 processing fee is required.

Your file will be reviewed upon receipt of your application and payment. Please be advised that additional documentation may be required, depending on the current state board requirements.
